The Department

The Aged Psychiatry Service (APS) provides assessment, treatment and ongoing management for people over the age of 65 years with a mental health disorder including psychological &
behavioural disturbances, for persons residing in the Inner South East catchment area.

The APS consists of Baringa which is a 15-bed acute inpatient unit as well as the community based Mobile Aged Psychiatry Service (MAPS), a hospital-based Consultation &
Liaison service (including off campus Transitional Care Program (TCP).

The Role

As one of the Senior Case Managers, you will be responsible for intake/triage, case management and/or consultation &
liaison.

You will have well developed organisational skills and will provide a high standard of case management to clients and expert liaison with other stakeholders in keeping with relevant legislation, policies and procedures. You will exhibit a high standard of clinical care delivery and be a role model to other junior team members and students.

Qualifications and Experience Required

Essential

  • Approved tertiary qualificationsand eligibility for membership with relevant registering body (Nursing and Midwifery Board / Occupational Therapy Board via AHPRA, or eligibility with AASW)
  • Postgraduate qualification(s) in Mental Health
  • Four years' experience post registration, to include clinical experience in acute psychiatric setting(s), and at least two years community psychiatric nursing experience
  • Experience and/or willingness to undertake telephone referrals and determine service response - desirable
  • Current Victorian Driver's License (without restriction)
  • Australian working rightsand full COVID-19 and Influenza vaccination
  • Be willing to undergo a Background Check
