The focus is on actionable insights that impact the health system in a positive way improving patient outcomes. This is done through 24/7 real time visibility end to end of on patient flow across the system, using existing data and shared insights to identify issues and developing a proactive response to support the system and patient care.

The SHCC uses real-time information systems, monitoring patients awaiting ambulances, people waiting for the ED, people waiting in the ED to be admitted, people waiting to be discharged, inter-hospital transfers and regional patients in metro hospitals, and a newly build real time communications platform to coordinate responses and actions.

The SHCC's role has developed in collaboration with the LHNs to deliver continuous improvement with a view to supporting LHN improvements and applying systemwide learnings and decision support tools around flow and access to care to deliver an improved patient journey.
