The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).

A Permanent Full time opportunity has become available to join our team as a Clinical Nurse Specialist Grade 2 (CNS2).
Working within a Primary Health Care framework and using a client centered and strengths based approach, the CNS2 for Chronic &
Complex Care will provide primary care;
care navigation, and care coordination to identified clients.

This exciting position will aim to p artner with clients, carers, and other services to comprehensively understand the client's needs and assist in goal setting and vital to this role is l iaising and networking with key stakeholders both internal to Primary Care and Community Health (PCCH), the Local Health District (LHD) and externally with General Practitioners (GPs), Specialists and the broader community sector.
As the Clinical Nurse Specialist you will endeavour to s trengthen working relationships, partnerships and referral networks to facilitate the integration of systems and processes that will inform the coordination of comprehensive and collaborative care to the target client group. The incumbent will be rostered across all programs and clinical areas and this may also include working weekends and public holidays.A uniform will be provided and required for this position.

What you will bring to the role:

  • Current Registered Nurse Division 1 National Registration (AHPRA) with a minimum of three (3) years full time equivalent post registration experience in a specialty area of practice related to the management of clients with chronic conditions who have complex care needs and a related recognised post graduate clinical qualification (Graduate Certificate level or equivalent) or such other qualifications or equivalent experience deemed appropriate.
  • Demonstrated advanced clinical skills and contemporary knowledge of evidence based Chronic Care Nursing practice and NSW Ministry of Health policies and initiatives for Chronic &
    Complex Care.
  • Demonstrated evidence of being able to provide person centred care and an ability to liaise with other health professionals to facilitate care navigation and care coordination.
  • Demonstrated ability to work independently with minimal supervision whilst maintaining involvement within the wider multidisciplinary team and with other service providers including GPs, Community Services and NGOs.
  • Demonstrated problem solving and decision making abilities and high level communication, liaison, negotiation and time management and report writing skills
  • Demonstrated ability to provide leadership in the provision and development of clinical practice and a demonstrated commitment to own professional development.
  • Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications and commitment to undertake training in electronic health record systems.
  • Current Driver's Licence and willingness to use for work purpose travel
