Clinical Nurse Consultant - ACP Registry

Metro South Palliative Care Service are looking for a Clinical Nurse Consultant to join their team. This department provides information and support for advance care planning (ACP) across the state, receiving ACP documents from all care environments in Queensland. Documents are then reviewed, and effective ACP documents are uploaded to The Viewer to make these important patient documents accessible to clinicians when needed. The Clinical Nurse Consultant will provide clinical expertise and consultancy to patients and clinicians and reviews ACP documents in accordance with relevant legislation.

About the Role

The purpose of this role is to provide clinical expertise and consultancy through advanced clinical leadership relevant to the speciality of advance care planning. The Clinical Nurse Consultant (CNC) provides direct and indirect input to patients and clinicians and reviews ACP documents in accordance with relevant legislation.

  • Responsible for high level clinical review of ACP documents through the application of advanced clinical and relevant legislative knowledge, skills and competency according to contemporary practice and standards of care.
  • Collaborate with the Office of ACP clinical and administration team and other members of the health care team to develop, implement, translate and evaluate clinical practice standard guidelines, policies, procedures and protocols using an evidence-based approach to clinical care and practice.
  • Provide patient and staff education, leadership within their expertise and act as a clinical resource, mentor, role model, and technical expert within the health care team environment.
